The fix was released last evening and the issue is now resolved. Impacted sites may continue to receive email notifications from Ocean regarding the extra message events that cannot be successfully delivered for up to four days (as described in this support article).

March 17, 2025 5:19 PM EST: We have determined the root cause of this issue. It related to a change released on March 13, 2025 to integrate with the Ontario Health PCCG. We will release a fix this evening to revert to the previous behaviour until it can be scoped more precisely to the PCCG integration scenario.
March 17, 2025, 4:25 PM EST: We are investigating reports that as part of the Ocean release on March 13, 2025 Ocean is sending an extraneous 'send-communication-from-provider' message event to FHIR integrated systems along with the 'send-communication-from-requestor' message event. This is impacting v0.10.0 and v0.11.1 integrations.
