Upcoming Enhancements: Support for 'Copy-to' Providers and eForm Response Retraction

Ocean will be making the following changes to the FHIR resources in the HL7 FHIR eReferral payloads. While these changes are backwards compatible with the existing implementation, integrators are encouraged to test their integrations to ensure compatibility with their parsing and business logic.

'Copy-to' providers

Ocean referral senders have the ability to 'carbon copy' (or 'copy-to') other providers on the referrals they are sending so that they are informed of the progress and outcome of the referral. The sender selects within the Healthmap UI one or more listings that represent the providers that will receive a carbon copy. See this support article for more details on this functionality.

Ocean will now include in the HL7 FHIR v0.11.1 payload an extension in the ServiceRequest resource for each provider that is carbon copied. The extension will include a reference to the PractitionerRole which would reference the Practitioner resource and Location resource. (Note that the Practitioner resource will only be included when present in the listing.) The format of an example ServiceRequest resource with the new extension is attached at the bottom of this support article.

New flag property in the QuestionnaireResponse resource

The QuestionnaireResponse resource corresponds to the output of the Ocean referral form completed by the referral sender. For each item that the sender answers in the form, Ocean includes the form item ID (LinkedID), the text of the questions (.text) and response (.value).

Where the sender has initially responded to a form item and has subsequently retracted, Ocean does not currently with-hold the initial answer from the QuestionnaireResponse resource.

A new extension will be added to the QuestionnaireResponse of v0.11.1 and v0.10.0 payloads that will indicate each form item response that has been hidden, including the retracted responses. The 'valueString' property will always be set to '"hidden" for this extension.  Below is an example:

Final Notes

Both of these changes are additive to the existing Ocean FHIR API integration and are backwards compatible with Ocean's existing APIs. Out of an abundance of caution, Ocean will make these changes available in the test.cognisantmd.com environment until Jan 9, 2025 so that integrators can test and mitigate any impacts to their existing integrations.
